About this calculator
The CKD-EPI 2021 formula is the updated estimated glomerular filtration rate (eGFR) equation for adults aged 18+, recommended by NKF-ASN in 2021 to replace the 2009 version. The key change is removal of the race coefficient, which previously overestimated eGFR in Black patients and delayed referral to transplantation and dialysis. Used in nephrology, cardiology, endocrinology, oncology, and primary care for chronic kidney disease staging, monitoring kidney function trends, nephrotoxic drug dose adjustment, and renal replacement planning. Inputs: serum creatinine, age, sex. CKD stages by eGFR (mL/min/1.73 m²): G1 ≥90 (normal or elevated), G2 60–89 (mild decrease), G3a 45–59 (moderate), G3b 30–44 (moderately severe), G4 15–29 (severe), G5 <15 (kidney failure). The formula is inaccurate at extremes of body mass, acute kidney injury, pregnancy, and in adults over 80 – inulin clearance or cystatin C (CKD-EPI cys 2012) provide more precise eGFR when needed.
