About this calculator
Pediatric analgesic-antipyretic dose calculator covers paracetamol and ibuprofen – first-line agents for fever and pain in children. Used by pediatricians, primary care physicians, emergency medicine specialists, and pediatric ward nurses. Paracetamol: 10–15 mg/kg per dose orally every 4–6 hours, up to 4 doses per day, maximum daily dose 60 mg/kg (not exceeding 4 g in adolescents). Approved from birth. Ibuprofen: 5–10 mg/kg per dose orally every 6–8 hours, maximum daily dose 30 mg/kg (not exceeding 1.2 g in adult formulations). Approved from 3 months. The calculator uses the child's body weight and converts the dose to milliliters of standard syrup (paracetamol 120 mg/5 mL, 250 mg/5 mL; ibuprofen 100 mg/5 mL, 200 mg/5 mL). Paracetamol is contraindicated in severe hepatic failure; in overdose (>150 mg/kg single dose) N-acetylcysteine is indicated. Ibuprofen is contraindicated in peptic ulcer, severe renal impairment (eGFR <30), decompensated heart failure, varicella (risk of streptococcal superinfection and Reye-like illness in children under 16 with influenza or varicella – no confirmed exemption for ibuprofen, caution remains), and dehydration. Neonates under 1 month, preterm infants, and children with severe comorbidities require individualized dosing by the pediatrician.
