What is citicoline used for?

What are the side effects of citicoline?

Common side effects of citicoline (≥ 1 in 100): Headache, Insomnia, Nausea, Decreased or increased blood pressure, Hot flushes. See the Safety section for uncommon and serious reactions.

Who should not take citicoline?

citicoline is contraindicated in: Hypersensitivity to citicoline; Severe parasympathetic hypertonia; Pregnancy – due to insufficient data; Breastfeeding – due to insufficient data; Age under 18 – due to insufficient data. Full list in the Safety section.

Citicoline for acute ischemic stroke is not supported by international guidelines. Cochrane 2020 (Martí-Carvajal) pooled 10 RCTs in over 4000 patients and showed no improvement in functional outcome, survival, or neurological recovery versus placebo. The large ICTUS trial (2012, over 2000 patients) was negative. European Stroke Organisation ( 2023) and do not include citicoline in standard care.

Standard therapy for acute ischemic stroke in international practice includes thrombolysis, thrombectomy, antiplatelets, and risk factor control. Citicoline is not part of evidence-based therapy.

Post-stroke cognitive impairment

No recomendado

Citicoline is not supported for post-stroke cognitive impairment by international guidelines. Systematic reviews show heterogeneous results with limited effect size on neuropsychological tests and no convincing impact on daily activities. Therapy relies on cognitive rehabilitation, vascular risk factor control, and management of post-stroke depression.

Traumatic brain injury

Citicoline is not used for traumatic brain injury per international guidelines. The COBRIT trial (2012, Zafonte et al., over 1200 patients) showed no improvement in functional and cognitive recovery in moderate-to-severe TBI versus placebo at 3 and 6 months. 2017 does not recommend citicoline for TBI.

Vascular dementia

No recomendado

Citicoline is not used as standard therapy for vascular dementia and cognitive impairment in patients with chronic cerebral ischemia. Evidence base is limited to small RCTs with methodological flaws and does not show clinically meaningful effect. Vascular dementia management relies on vascular risk factor control (hypertension, diabetes, dyslipidemia), antiplatelets, and cognitive rehabilitation.

Cuándo y cómo tomar

In Russia the drug is available as oral solution, 500 mg tablets, and parenteral 1000 mg IM or IV. If ineffective after 6–12 weeks, continued use is not justified. Not used in international guidelines, so no standardized regimens exist.

Monitorización

No specific monitoring required. Efficacy is assessed clinically. If no clinically meaningful improvement after 6–12 weeks, discontinue. Routine laboratory tests for therapy monitoring are not foreseen.

Situaciones especiales

The drug should not be used as a substitute for evidence-based therapy. Acute ischemic stroke relies on thrombolysis, thrombectomy, and antiplatelets; TBI on neurosurgical care and intracranial pressure management; vascular dementia on control of blood pressure, diabetes, dyslipidemia. Adding citicoline should not delay or replace evidence-based treatment.

Mitos frecuentes

Myth: "Ceraxon treats stroke and restores the brain". Fact: large RCTs (ICTUS 2012, over 2000 patients) and the Cochrane 2020 review showed no improvement in acute ischemic stroke outcomes versus placebo. Recovery comes from rehabilitation, not citicoline.

Myth: "citicoline is a proven neuroprotector". Fact: international guidelines ( 2023, , ) do not include citicoline in standard care due to lack of evidence.

Myth: "Neuraxon improves memory in vascular dementia". Fact: vascular dementia management relies on control of blood pressure, diabetes, dyslipidemia; unproven neuroprotectors do not replace this approach.

Myth: "after TBI one needs course-based citicoline for recovery". Fact: the COBRIT RCT (2012) showed no improvement in functional and cognitive recovery in TBI. Recovery comes from rehabilitation.
