About this calculator
HOMA-IR (Homeostatic Model Assessment of Insulin Resistance) is the primary surrogate marker of insulin resistance in adults without type 1 diabetes. Used in endocrinology for metabolic syndrome workup, in gynecology for PCOS evaluation, and in primary care in patients with central obesity, non-alcoholic fatty liver disease, or hypertension. Formula: HOMA-IR = (fasting insulin µU/mL × fasting glucose mmol/L) / 22.5. Blood must be drawn fasting (8–12 hours). Reference ranges: < 1.0 – optimal sensitivity, 1.0–1.9 – normal, 2.0–2.9 – early insulin resistance, ≥ 3.0 – significant insulin resistance requiring lifestyle and/or pharmacological intervention. Method sensitivity is reduced in patients with severe β-cell dysfunction, so HOMA-IR is not applicable in overt type 1 diabetes.
