Gentamicin is used in combination with a beta-lactam or vancomycin at synergistic doses in native-valve infective endocarditis of enterococcal etiology. 2015 and 2023 recommend 3 mg/kg/day in 3 divided doses for 2-6 weeks depending on the regimen. In staphylococcal prosthetic-valve endocarditis, gentamicin is added during the first 2 weeks per ESC 2023. In viridans streptococcal endocarditis, a short 2-week course with penicillin is part of standard protocols.
Peak and trough levels are monitored: trough below 1 mcg/mL. In patients with renal impairment or ventilator-associated infection, dose and interval are adjusted to eGFR.
